Company Safety Violation Analysis Assignment Instructions
Find an article for a company that has had a major safety incident.
There should be 6 headings:
Introduction: What is the company and their description?  What do they produce? Where is it located?
Management Role: What role did management play in the situation? Which managers were accountable for the event. Explain why they were accountable. (Managers could also consist of team leads or specialist support staff, or a group of people).
Safety Issue and Rules: What happened? Why and where did the event occur? What was the safety issue and the rules that appeared to be violated? If no established rules were violated, what “normal” safety rules applied?
Metrics: Define if safety metrics were in place?  If not, what metrics should have been monitored? Which metrics need adding? Include at least five metrics and explain the reasons for each metric. Include who participates in examining each metric, which managers, which employees, and when.
Mitigation: What training would you recommend for the employees that would mitigate this safety risk? Why should they receive that specific training? Whom should give the training and by whom? When should training occur and how often? How should training be reported/recorded and who should get the information?
Conclusion: Summary
